Surrey Heath Clinical Commissioning Group (SHCCG ) is working in conjunction with North East Hampshire
& Farnham CCG to re-procure new GP Out of Hours elements of the urgent care
pathway. A questionnaire has been devised to obtain views from users of
the NHS 111 and Out Of Hours services. Frimley A&E and North Hampshire Urgent Care Out Of Hours service are encouraging
patients using their services between now and the 13th September to complete the following online questionnaire:

We are changing our
practice was started by Dr Sam Bartlett , shortly after the Second World War.
Since 1945, Ash Vale Health Centre and Frimley Green Medical Centre has been one large GP partnership working from
our 2 sites.
order to consolidate and improve the efficiency of the work we do in
providing a health service to our 28,000 patients we are going to shortly
combine our 2 IT systems. It seems the appropriate moment to go forward under a
single name and in honour of Dr Sam Bartlett and his 2 sons, Dr Gregor Bartlett
and Dr Callum Bartlett who together lead the practice for many years we will be
changing our name to:

Bartlett Group Practice
services will continue as before and access phone numbers will not change,
although, there will be new website introduced to reflect these new changes. We
actually anticipate improving our service because we will be able to work
smarter by working more closely together. There
may be some unexpected problems as our software suppliers seek to make the
transfer from two IT systems to one, so please bear with us through these
changes as we seek to provide a normal service throughout the changeover.
IMPORTANT – Due
to an essential IT merge with our practice at Frimley Green which took
place between the 17th - 21st June, previous users of EMIS online Patient Access are no longer
able to use any of the online services. Patients will need to request new online account
registration details by contacting Reception or emailing email@example.com . We
are sorry for any inconvenience but this IT upgrade now enables the surgery to
commence offering the Electronic Prescription Service to patients as well as
enabling the practice to meet new NHS contractual reporting obligations. We have around 2,500 active online users who individually
will need to be issued with new online registration details, unfortunately the
clinical IT systems provider, could not provide an automated solution to do this and
we would therefore please ask for your understanding and patience whilst we try
and process these requests as quickly as possible.
01.04.16 - NHS Prescription Charges - With effect from the 1st April 2016, prescription charges for each item dispensed will increase to £8.40. If you need 4 or more prescriptions in 3 months or 13 or more prescriptions a year, you could save money with a Prescription Prepayment certificate.

Join our 'Virtual' Patient Group If you would like to become involved with our Patient Representative Group and have some input into how the surgery runs and what services you feel the surgery should prioritise for it's patients, you would be very welcome. We currently have around 90 patients registered as members. Although the majority of communication is via email, you can still participate through the post if you prefer.
